Digitized wellness assessments

Paper-based management of health risk assessments are riddled with risks that compund a bigger individual health risk. Recording patients' health data manually on paper has the following risks.

  1. Human error
  2. Data loss
  3. Patient care inertia.

Our technology provides digital biodata measurements and recording. This eliminates incorrect recording of biodata (weight, blood pressure, glucose etc.) by the health professional. The digital recording provides integrity to the measured and recorded data. Secondly, the technology creates an electronic health record (EHR) for the individual examined. This eliminates the data loss risk.

Assessing the health risk

Care inertia occurs when health professionals/ organizations fail to follow-up on a high health risk patient in need of integrated health services. Telimpilo™ Healthcare has developed innovative care protocols for individuals identified as having health risks during a health risk assessment event. Our protocols ensure accurate patient diagnosis and proper data informed management.

Once identified, the patient is provided with a health monitoring device for self-measurement at home. Remote monitoring of the patient's data takes place, followed by data analytics which will confirm the level of the health risk and confirmation of the disease diagnosis.

Active health risk management

Two broad groups of high heath risk individuals are identified during a health risk assessment process.

  1. Undiagnosed individuals with uderlying disease.
  2. Diagnosed individuals on medication for treatment, with uncontrolled disease.

Both groups are at risk of morbidity and mortality from the underlying disease. Our innovative protocols require that these individuals be provided with respective self-measuring devices to use at home for a period of 2 weeks. Their biodata is remotely monitored, analyzed and the patient managed collaboratively.
